W9 Request

Please note that the HCAA requires a current-year W9 form to be on file before processing any payouts.

To submit your W9 please follow these instructions:

1. Click this link IRS Form W9 to access the official IRS W9 PDF form.

2. Fill out the form completely and accurately.

3. Save the completed PDF to your computer or device, making note of its location.

4. Return to this page and upload the saved PDF from the location on your device.

5. Once the file is uploaded, click "Submit" to complete the process.
